Physician Support
The primary goal of each Alere disease management program is to encourage, support and reinforce the participant’s compliance with their physician’s plan of care. Care managers review the physician’s plan of care with the participant early in the program and determine if the participant has a thorough understanding of care and addresses any barriers to compliance. The process of reinforcing the physician’s plan of care with the participant takes place during every interaction and is a core element of Alere's personal health support approach.
Our physician engagement strategies are designed to keep treating physicians informed about their participants’ progress as well as any care gaps or risk management issues. This occurs in parallel with ongoing efforts to educate participants about when to contact their physician, how to prepare for a visit and what care gaps require physician involvement (e.g., testing or prescriptions) to attain compliance.
To foster effective communication and follow- up relative to each participant’s level of program participation or lack thereof, Alere provides participant-level reports to physicians. The following provides an overview of communication reports and modality.
